Senior Outreach Coordinator

SENIOR SERVICES RURAL OUTREACH CASE MANAGER - 40 hours weekly

LCCC is seeking to hire a contract full-time Rural Outreach Case Manager to provide outreach and much-needed resources to the most isolated seniors of Livingston County.

The position would provide part-time staffing for the Fowlerville Senior Center, a rural and underserved area, to provide health and wellness activities; geriatric depression and substance use screenings; and resources to local seniors. The second portion of the position is to work closely with the LCCC Senior Services Community Liaison team to provide geriatric depression and substance use screenings in Livingston County, along with education and resources, through senior centers, local churches, and other organizations with a focus on seniors located in the most rural townships in Livingston County. BSW or similar degree preferred.
